Terms & Conditions
Thank you for choosing to work with us on your recovery journey. These Terms and Conditions outline the agreement between you (the "Client") and Melissa L. Moore, LLC. By engaging with my services, you acknowledge that you have read, understood, and agree to be bound by these terms.
Recovery coaching services are designed to provide support, accountability, guidance, and skill-building resources to individuals seeking recovery from substance use disorders and related challenges. Services may include but are not limited to one-on-one coaching sessions, group coaching sessions, educational materials, and access to community resources.
The coaching services provided are NOT a substitute for professional medical, psychological, or psychiatric treatment. Recovery coaching is a non-clinical service that complements, but does not replace, professional healthcare services.
The Coach is not a licensed healthcare provider, therapist, counselor, or addiction treatment specialist unless explicitly stated otherwise. The Coach does not diagnose conditions, prescribe medications, or provide psychotherapy.The Coach does not provide emergency services. In case of an emergency, please call 911, go to your nearest emergency room, or contact the National Suicide Prevention Lifeline at 1-800-273-8255.
~The Client agrees to actively participate in the coaching relationship, which includes:
~Being honest and forthcoming about challenges, progress, and setbacks.
~Completing agreed-upon actions between sessions to the best of their ability.
~Providing feedback about what is and isn't working in the coaching relationship.
~Taking responsibility for their own choices and actions.
The Client is responsible for:
~Maintaining their physical and mental health through appropriate professional careInforming the Coach of any changes in health status that might affect the coaching relationship.
~Following medical advice from qualified healthcare providers.
~Ensuring they have appropriate clinical support if needed.
The Client acknowledges that successful recovery requires consistent effort and commitment. Results vary based on individual circumstances, engagement level, and personal effort.
Appointments are scheduled based on mutual availability. The Coach will make every effort to accommodate the Client's preferred times.
The Client agrees to provide at least 24 hours' notice for cancellations. Late cancellations (less than 24 hours' notice) or missed appointments may be subject to the full session fee at the Coach's discretion.Exceptions may be made for genuine emergencies at the Coach's discretion.
If the Client arrives late to a session, the session will still end at the scheduled time. The Coach will wait up to 15 minutes for a Client who is late before considering the appointment missed.
Coaching fees are as outlined in the service agreement or on our website. The Coach reserves the right to adjust fees with appropriate notice.
Payment is due at the time of service or as specified in the service agreement.Accepted payment methods include Credit Card, Zelle, PayPal, Venmo, Checks. For package purchases, payment is due in advance unless a payment plan has been arranged.
Individual sessions are non-refundable once completed. Package purchases may be partially refundable for unused sessions, less an administrative fee of 25%. Refund requests must be submitted in writing and will be evaluated on a case-by-case basis.
The Coach will maintain the confidentiality of all Client communications and information, subject to the limitations described below.
Confidentiality may be broken under the following circumstances:
If the Client presents a danger to themselves or othersIf there is reasonable suspicion of abuse or neglect of a child, elderly person, or dependent adultIf required by law or court orderWith the explicit permission of the Client
Our privacy practices comply with applicable laws and regulations. Please refer to our Privacy Policy for details on how we collect, use, and protect your personal information.
The Coach maintains basic records of sessions to ensure continuity of care. These records are kept secure and confidential.
The Coach-Client relationship is a professional one. The Coach will not engage in personal, financial, or romantic relationships with Clients during the coaching relationship or for a period of two years afterward.
The Coach's policy regarding social media connections with Clients is: The Coach does not accept friend requests from current or former Clients on personal social media accounts but maintains a professional page where Clients may connect.
The Coach is available for scheduled sessions and brief check-ins as outlined in the service agreement. The Coach is not available for extensive communication between sessions except in cases of genuine urgency.
The Client may terminate coaching services at any time by providing written notice.
The Coach may terminate services under the following circumstances:
~If the Coach determines that the Client would be better served by another professional
~If the Client is not adhering to the program requirements
~If the Coach-Client relationship has become ineffective
~If the Client exhibits abusive or inappropriate behavior
~If the Client fails to pay fees as agreed
When possible, termination will include:
Discussion of the reasons for termination
~Summary of progress made
~Referrals to alternative resources as appropriate
The Coach will respond to messages within [timeframe, e.g., 24-48 business hours]. The Coach is not available 24/7 and does not guarantee immediate responses.

The preferred methods of communication are phone call, texting, or email.
All materials provided by the Coach, including worksheets, recordings, exercises, and other resources, are the intellectual property of the Coach or appropriately licensed for use.
The Client may use these materials for personal use only and may not reproduce, distribute, or share them without explicit permission.
The Coach makes no guarantees or warranties, expressed or implied, about the results or outcomes of the coaching services.
The Coach's liability is limited to the amount paid for coaching services. The Coach is not liable for indirect, consequential, or special damages.
The Client agrees to indemnify and hold harmless the Coach from any claims, damages, or liabilities arising from the Client's actions or decisions.
The Coach reserves the right to amend these Terms and Conditions at any time. Changes will be communicated to the Client and will be effective 30 days after notification.
Continued use of coaching services after amendments constitutes acceptance of the new terms.
These Terms and Conditions are governed by the laws of state of Illinois.
Any disputes arising from these Terms and Conditions will be resolved through:
~Good faith negotiation between the parties
~Mediation with a mutually agreed-upon mediator
~Binding arbitration if mediation is unsuccessful
By engaging with our coaching services, the Client acknowledges that they have read, understood, and agree to these Terms and Conditions. The Client also acknowledges that recovery is a personal journey, and results depend on many factors, including the Client's commitment and active participation.
